UPDATE 1/7/19
Taking a moment from the day to acknowledge Felikian's for contacting me and stopping by to inspect the floors. Their willingness to contact a single customer install from 2016 shows concern beyond a tailgate warranty.
The service manager stated that he will order new floors from the manufacturer and gave me the option of bringing out a whole new crew to remove as many floors as the situation needs to level out the slope and address all the problem areas.
I don't think a customer can ask for anything more.
Thank you.

I've made it a point not to throw virtual rocks through the windows of local establishments by giving them a 1 star. Although I would like to considering the money we spent. But I cannot give them a higher rating based on how my install is holding up.
Originally purchased the H20 Engineered Luxury Vinyl Floors back in Feb 2016. The install has never really been without any issue other than the first few weeks after install. The issues were not major but one would think something like floors should not be problematic for at least 5 years. Little gaps in various places. Didn't bother me much until my wife started pointing them out.
Within a year of the install, Felikian's was good about coming back out and "fixing" the problem. Not really much they can do about the gaps besides pounding the planks with their mallets and using a bunch of adhesive to try ti fill the gaps and keep it from spreading.
Fast forward to 2018. Been seeing gaps all year but occupied with more pressing life matters, I never got around to doing anything. That is until my wife started pointing them out again. Few gaps that were fixed became noticeable again. Baseboards are detaching from the walls. Easy fix but should I really be the one fixing it?
The biggest issue I would have to say is going to be in my den. I say going to be because I know I have not seen the worst of it. My den was an addition like all additions in the 70's, kind of funky. There is a slight slope transitioning into this den. The installers thought it would be a good idea to use the boxes to reduce the slope and install the floor on top of it. The result is a squeak when stepped on and a slope that is a bit slippery. My kids love the spot as they get to slide over it. Part of the increasing slope is probably due to the cardboard losing its integrity. Maybe if I was told the slope was too severe and would need to be leveled out prior to install, something could have been done about it at that time. I can only speculate why they decided not to inform me.
If I were to guess why my installation is not holding up as it should, the install was not done as directed by the manufacturer. Everyone has seen a floor install here and there. You place a wooden piece like a 2x4 against the floor plank and pound it into place with a mallet. The installer however, just used a mallet and pounded the floor planks into place using the impact and friction of the rubber mallet. I did mention it, but he said it's fine.
Ultimately, the issues with my floor are inconsequential. My house is old and it's not the first thing you see when you walk through the front door.
If you choose this place, I would recommend getting a price for the floors and find your own expert installers.
