He should have printed off the form and sent it in. It has not been ordered. There are better places locally for optical needs. ANY place is better than this.

My glasses broke at work and I went to Hakim and ran to the door and they were closed because the lady was going on break!! She saw me as she was driving away and drove back into the store and opened the door and let me in !!!!! Not only did she find me the exact frame but she switched my lenses and adjusted my second pair in less than 15 min!!
