I would like to give the facility five stars. My mother attended rehab at the facility after being in Sisters Hospital for almost one month. The team of professionals cared for my mother in a very caring and professional manner. The team of professionals addressed all of my mothers concerns as well as addressing her five daughters concerns. The food was accommodating for Moms Reno diet as well as for diabetes diet. When my mother's term was complete she was healthy and was prepared to return home. We as a family appreciated the care that was provided for our mother.

They are terrible!!! Very inconsiderate, half the time the aide doesn’t show up and when she does, she plays on her phone most of the time. There is a list of things she is suppose to do hanging on the refrigerator and half the stuff never gets done.

Not what I expected! The Company is unorganized, few times the back up Aide was a no show or never answered call when tried to b reached from the company. The Company never consider 2 call the family member of the Aide unable 2 arrive. My mom is disabled & need assistance. Had 2 call family member @ last minute 4 back up.

My son-in-law was there after surgery. It's beautiful and so clean! The food was edible and the staff was super nice. They even tried to help extend his stay, let him stay a couple more days when his insurance ran out and he was recommended to stay two months, not 3 weeks. If anything happens to me, this is where I'm going!

This agency don't like to pay their employees, very unprofessional and unorganized. Have left many of their priority clients without services. Pays minimum wage. Make you buy your own gloves, does not offer incinitives when they ask you to pick up a case last minute.

We were blessed to be able to care for our mom at home until she was 88. We were able to achieve this through enrollment in the PACE program when she was 84. Our mom thrived here! She enjoyed the activities, was comfortable with the staff and had wonderful dedicated nurse practitioners, nurses, and aides who grew close to her. We gathered with our family to say our final goodbyes this past July. This group of caregivers cried with us. They were like family! We were blessed and grateful that mom transitioned very peacefully. Whenever we had questions or concerns, they were addressed. Our hearts were heavy when mom had fallen and we could no longer keep her home, but through OLV and the Mercy Nursing Facility, we were given another year-and-a-half with our mom! God bless all of these professional dedicated caregivers for this gift to us! We remain sincerely and humbly grateful!

Very loving home. My Mom only lived there for 3 Months before her passing every day she was loved by one staff member or another. Sure some are there only for a job but I would say most employees love their residents, Thank you for you love and care of my Mom.

my aide is great. we get along fantastically but as for health force, one or two of the scheduling coradinators are not so nice. one even said to me why do you keep making your doctor appointments on the day your aide comes. and i said well its not my fault that some of my doctors only work on certain days. good thing i am a nice person and did not get rude back. when I started having a aide 1 year ago, I was told by the head nurse at the time, that me and my aide where allowed to go for walks, go to the store, even go to wendeys for lunch, *********** as long as we did not pay for eachother or loan eachother money. now the new head nurse says we cant do that anymore that it was never on my care plan. and now he claims its a insurance thing. i am stuck in my apt in the winter 24/7 from dec until may. when the good weather hits me and my aide thought great we can go out, shop for food, maybe grab a sandwhich and bring it back and sit outside and eat our lunch. I feel like a prisoner in my own home. my aid is here m,w,f, and tues and thursday rehab. I am very close to telling my casework, I just feel like saying heck with it, and give up health force. and maybe even not get a aide any more at all. i want this straightened our, i would hate to lose my aide.
